Below please find a variety of forms for your use:

• POINSETTIA ORDER FORM: If you’d like to purchase a Christmas poinsettia to decorate the altar during Christmas Eve worship services, please download and complete an order form and return it — along with a check made payable to The First Congregational Church of Western Springs — to the church office by December 11. Be sure to note “POINSETTIA” in the memo line. Poinsettia plants cost $17.00 each.

• GRANT APPLICATION: Any registered 501(c)(3) organization is invited to submit a completed application in the fall of an odd year for review. • PF & CONFIRMATION MEDICAL FORMS: If your child is a member of PF or Confirmation Class, please complete the appropriate medical form:

• FAITH & FLOW YOGA WAIVER: If you would like to participate in our Faith & Flow Yoga program, please download, complete and return a copy of the following waiver to hello@wscongo.org:

• TAI CHI TUESDAY WAIVER: If you would like to participate in our Tai Chi Tuesday program, please download, complete and return a copy of the following waiver to Faith Community Nurse Jean Larson at jean@wscongo.org:

• ROOM SETUP FORM: A Room Setup Form is required when requesting a room reservation for an event or meeting. Please download a copy by pressing the button below. Email completed forms to beth@wscongo.org.

• WORK ORDER FORM: A Work Order Form is required when requesting a repair for anything on our campus.   Please download a copy by pressing the button below. Print the completed form, make a copy for yourself, and put a copy in the mailboxes of Rich Kirchherr and Scott Williams.

• PAYMENT VOUCHER: A Payment Voucher is required when requesting reimbursement for expenses. Please download a copy by pressing the button below. Email completed forms to nancy@wscongo.org
